VACCINATION REQUIREMENTS
Fidos boarding kennels requires proof of current vaccinations to ensure the health of all pets staying with us. (C5: dogs, f3: cats)
If you do not have a copy of your pet/s current vaccinations, please provide us with your veterinarians name and number and we would be happy to contact them on your behalf.

If your pet/s vaccinations cannot be confirmed by the time of check in we cannot accept your pet/s.

AUTHORISATION FOR MEDICAL TREATMENT
If your pet/s become ill during their stay with us, we will make every attempt to contact you on the emergency number provided on the front of this form and we will have a veteriniaran examine your pet/s. Medical treatment will be provided at the discretion of the veterinarian and all costs will be at the owners expense.

AUTHORISATION FOR RELEASE OF YOUR PET
If someone other than yourself (the owner) is to pick up your pet/s from fidos boarding kennels, please let us know when you bring your pet/s in. Fidos boarding kennels will not release any pet/s to someone other than yourself (the owner) without prior authorisation.

FLEA AND TICK TREATMENT
Your pet/s are required to have a flea and tick treatment that is current for the length of their stay. This can be either frontline plus, advantix, nexguard or bravecto. If your pet/s require a flea / tick treatment, you may bring your own or they are available from the office at a cost of $15 each. Please inform us if this needs to be administered during their stay.

YOUR PETS BELONGINGS
While we do our best to return all items left with your pet at the time of discharge, occasionally items can be misplaced. Fidos boarding kennels does not accept responsibility for the loss of any such belongings that may be misplaced while your pet is boarding with us.

However, if you would like to leave any of your pets personal belongings with us, please provide a list below and ensure your pets name is marked clearly on each item/s.

The undersigned acknowledges contracting for the above services and undertsands that any / all costs incurred will be the responsibility of the pet owner, and he / she is responsible for all balances due. These conditions will apply each time pet/s are admitted to fidos boarding kennels.
